1.0 PURPOSE

The intent of this Invitation for Bid is to hire one Bidder to provide dental hygiene consultation & services to the nine (9) Western Tidewater Community Services Board (WTCSB) residential homes located in Suffolk and Zuni. The WTCSB anticipates awarding a one-year contract with up to three (3) additional one-year renewal options.

2.0 COMPETITION INTENDED

It is the WTCSB's intent that this Invitation for Bid (IFB) permits competition. It shall be the Bidder's responsibility to advise the Buyer in writing if any language requirement, specification, etc., or any combination thereof, inadvertently restricts or limits the requirements stated in this IFB to a single source.

3.0 MINIMUM QUALIFICATIONS

3.1       The Bidder shall supply all items necessary to perform the described services.

3.2       The Bidder must be a fully qualified Dental Hygienist, licensed to practice in the Commonwealth of Virginia, and in good standing with the Virginia Department of Health Professions Board of Dentistry.          

3.3       The Bidder shall provide a list of at least three (3) current references for whom comparable work has been performed in the space provided on the BID FORM.

3.4       The Bidder shall provide a current copy of the License to Practice as a Dental Hygienist from the Virginia Department of Health Professions Board of Dentistry.

3.5       The Bidder shall provide proof of Automobile & Professional Liability Policies as set forth herein. 

4.0 SCOPE OF SERVICES

4.1       The Bidder shall be responsible for the following:   

Monthly surveillance and evaluation of each resident’s progress and/or maintenance in oral hygiene habits to occur at each of the nine (9) residential homes.

Consultation to staff regarding proper methods to maintain acceptable oral health.

Recommendations for dental care or treatment when appropriate.

Monthly review of dental records.

Document in the individual resident record all services rendered under this agreement.

Participation on the Infection Control Committee via zoom or submitted report. 

Attend monthly Interdisciplinary Team Meetings via zoom or submitted report.

All services at each residential home will be at a time mutually agreed upon by the Bidder and Medical & Nursing Practices Supervisor. 

If the Bidder is unable to perform the work in the time required, the WTCSB reserves the right to obtain services from another qualified Contractor.

It is understood and agreed that the WTCSB will not guarantee any minimum amount of work. Any contract resulting from this solicitation shall obligate the WTCSB to utilize the successful Bidder for the services described. Should the Bidder fail to respond within the designated time, the WTCSB reserves the right to secure services from other sources; the Bidder shall be liable for any additional expense.

The Bidder shall submit a monthly invoice to AP@wtcsb.org. Payment will be made net thirty (30) days upon receipt of invoice. All invoices must be submitted within forty-five (45) days following completion of job.
