Requirements:

• DDS or DMD accredited by the Commission on Dental Accreditation 
• Certificate in Pediatric Dentistry
• Current, valid license to practice in Tennessee or be eligible for licensure
• Other certification required by TN to include state specialty license, sedation permit, DEA, CPR, PALS
• No sanctions by Medicaid, Medicare, or any licensing board
